The Suffolk Recording & Endorsement Form, RP-5217 and the Peconic Bay CPF form must be printed on legal size paper (8½ × 14) or the clerk will reject the package. RP-5217 has to be downloaded and opened in Adobe Acrobat Reader — it will not fill in correctly in a browser.

Peconic Bay towns

The CPF form is required only for East Hampton, Riverhead, Shelter Island, Southampton and Southold.
